In-Person Performing Art, Musical Theatre, Dance Class Waiver
I, PARENT/GUARDIAN OF SAID CHILD(REN) HEREBY ASSUME ALL OF THE RISKS OF MY CHILD(REN) PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH IN PERSON PERFORMING ART/MUSICAL THEATRE/DANCE CLASSES/THEATRE FIELD TRIP OUTING, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective reigns as follows:
(A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of my child(ren), for my child(ren)'s death, disability, personal injury, or actions of any kind which may hereafter occur to my child(ren) during IN PERSON PERFORMING ART/MUSICAL THEATRE/DANCE CLASSES/THEATRE FIELD TRIP OUTING BY THE FOLLOWING ENTITIES OR PERSONS: HaoIs International Inc. Haois International Arts School and/or their directors, employees, or volunteers.
(B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE THE FOLLOWING ENTITIES OR PERSONS: HaoIs International Inc. Haois International Arts School and/or their directors, employees, or volunteers from any and all liabilities or claims made as a result of participation in IN PERSON PERFORMING ART/MUSICAL THEATRE/DANCE CLASSES/THEATRE FIELD TRIP OUTING whether caused by the negligence of release or otherwise. I acknowledge that HaoIs International Inc. Haois International Arts School and their directors, employees, and volunteers are NOT responsible for errors, omissions, acts, or failures to act of any party or entity conducting IN PERSON PERFORMING ART/MUSICAL THEATRE/DANCE CLASSES/THEATRE FIELD TRIP OUTING on their behalf. I acknowledge that this activity may involve injury. The risks include, but are not limited to, those caused by jumping, leaping, turning, stretching, or any dance movement, lack of hydration, improper dance floor, and actions of other people including, but not limited to, participants, and/or producers of the activity.
(C)The PARENT/GUARDIAN of Student furthermore warrants that the student has no known medical condition(s) which will prevent him/her from undertaking dancing, singing and acting and so on performing arts musical theatre learning related activities safely and will not hold HaoIs International Inc. Haois International Arts School and their directors, employees, and volunteers liable for any injuries he/she sustains as a result of any medical conditions he/she may have, pre-existing or otherwise.
I hereby consent for my child to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during IN PERSON PERFORMING ART/MUSICAL THEATRE/DANCE CLASSES/THEATRE FIELD TRIP OUTING. I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT. I SIGN IT OF MY OWN FREE WILL. I certify that my child(ren) is physically fit, has sufficiently prepared or trained for participation in IN PERSON PERFORMING ART/MUSICAL THEATRE/DANCE CLASSES/THEATRE FIELD TRIP OUTING, and has not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my child(ren)'s participation in IN PERSON PERFORMING ART/MUSICAL THEATRE/DANCE CLASSES/THEATRE FIELD TRIP OUTING. I acknowledge that this Accident Waiver and Release of Liability Form will be used by HaoIs International Inc. Haois International Arts School, the director, class holders, sponsors, and organizers of the activity in which my child may participate and that it will govern my child(ren)'s actions and responsibilities at IN PERSON PERFORMING ART/MUSICAL THEATRE/DANCE CLASSES/THEATRE FIELD TRIP OUTING. I permit my child to participate in IN PERSON PERFORMING ART/MUSICAL THEATRE/DANCE CLASSES/THEATRE FIELD TRIP OUTING EVENT.
COVID-19 Protocols
I understand that strict safety measures and public health guidelines have been implemented at HaoIs International Inc. HaoIs Musical Theatre Performing Arts School to ensure the safety of all theatre families, staff and the community.
All protocols, including Drop-off and Pick-up procedures, are listed on our website.
Reduced Studio Capacity: Class sizes have been reduced to comply with physical distancing measures.

Tuition Fees & Refund Policy
TUITION FEES & PAYMENT OPTIONS: New online registrations as of 2020 will be processed via 'E-Payment' - through Email transfer.
A Registration Fee of $20.00 will be charged to new families upon registration. This fee is one-time and NON-REFUNDABLE.
Tuition Fees must be paid in full prior to all In-Person classes in order to be allowed to participate in the education program, and to reserve your spot, as spaces are limited.
- REFUNDS/WITHDRAWALS: All withdrawals must be submitted in writing to the Musical Theatre Program Coordinator, Helen Hao Wang. Email address: [email protected]
Withdrawals will be taken as of the date the office receives written notice.
Withdrawals from any Weekely Classes, Camp programs must be made at least two weeks prior to the start date for a full refund, minus our registration fee for new families of $20.00. If a Withdrawal form has been received within two week prior to the start date of the class program, PARTIAL REFUNDS of 50% will be issued: an additional $20 Administration Fee per person will be deducted to process the refund. If a Withdrawal form has been received after the Class Program started, NO REFUNDS will be issued.
SCHOOL CLOSURE: In the event of a government-mandated closure, all classes will be offered virtually.
Recitals & Costumes
Due to the current COVID-19 pandemic, plans for our showcases will be confirmed as the program progresses. In the event that performances are possible, students will be given the option to participate. Those who choose to perform will require a costume for showcases. Costumes will be ordered by Haois International Arts School, costume fees will be discussed with families in advance.
Class Cancellation
HaoIs International Inc. Haois International Arts School reserves the right to cancel any class/program temporarily or permanently due to emergency, weather conditions, or low enrollment. The cancellation notice will be emailed to all registered students and posted on our website, Instagram and Facebook page. Please ensure that you have provided a valid email address with your registration in order to keep up with communication.
In the event of a government-mandated closure in the province of Ontario due to COVID-19 or other emergency situation where we are forced to temporarily close, all programs will continue Virtually.
Dress Code Policy
All students are expected to comply with the dress code requirements. These may be found in your Welcome email or with HaoIs staff at any time.
Communication
Important Musical School information is distributed and updated on a regular basis. Our main means of communication is via WeChat, Whatsapp, and email. Please check for updates regularly to avoid confusion. Please ensure your email address is correct upon registration.
Permissions
Permission is granted for my child to be photographed/video recorded on HaoIs International Inc. Haois International Arts School premises and to be photographed/video-recorded as part of, but not limited to, live Zoom virtual classes. All Zoom classes will be recorded. Permission is also granted that any of these photographs or videos may be used as marketing material to promote HaoIs Musical Theatre Performing Arts School Kids Program.
Miscellaneous
Please leave any valuables at home, the studio is not responsible for any lost or stolen articles.
For safety reasons please avoid jewellery and ensure that you are dressed appropriately in comfortable clothing and indoor athletic shoes to allow for proper instruction.
Parents must pick up their children immediately following their class.
Acknowledgement and Assumption of Risk
On behalf of myself or my child(ren), I hereby acknowledge and agree that:
Dancing, singing, acting and theatre field trips may be dangerous, exposing participants to many risks and hazards, some of which are inherent in the very nature of dancing, singing and acting itself, others which result from human error and negligence on the part of the persons involved in preparing, organizing and staging the dancing, singing, acting and theatre field trips;
As a result of the aforesaid risks and hazards, my child as a participant may suffer serious personal injury, even death, as well as property loss;
Some of the aforesaid risks and hazards are foreseeable, but others are not;
I nevertheless FREELY AND VOLUNTARILY ASSUME ALL THE AFORESAID RISKS AND HAZARDS, and that, accordingly, my preparation for, and participation in dancing, singing, acting and theatre field trip outing and so on performing arts musical theatre learning related activities SHALL BE ENTIRELY AT MY OWN RISK;
I understand that neither HaoIs International Inc. Haois International Arts School, nor any of its directors, officers, employees, sponsors, independent contractors, members, players or agents assume any responsibility whatsoever for my safety during the course of my preparation for or participation in dancing, singing, acting and theatre field trip outing and so on performing arts musical theatre learning related activities;
I have carefully read this ACKNOWLEDGMENT AND ASSUMPTION OF RISK FORM , fully understand same, and acknowledge that I am freely and voluntarily executing this Form
I have been given the opportunity and have been encouraged to seek legal and parental advice prior to signing this Form;
I clearly understand that HAOIS INTERNATIONAL INC. HAOIS INTERNATIONAL ARTS SCHOOL would not permit me to participate in dancing, singing, acting and theatre field trip outing and so on performing arts musical theatre learning related activities unless I signed this ACKNOWLEDGMENT AND ASSUMPTION OF RISK FORM, and that this ACKNOWLEDGEMENT AND ASSUMPTION OF RISK FORM applies to the dancing, singing, acting and theatre field trip outing and so on performing arts musical theatre learning related activities and that the terms of this Form have been explained to me by HaoIs International Inc. Haois International Arts School or one or more of their representatives; and I have read the above and agree.

An exception will be made in the case of medical emergency reasons (doctor’s note must be provided) or relocation.
- There is no refund or credit for missed classes.

OUR COMMITMENT TO YOUR SAFETY
At Haois International Arts School, the safety of our students, teachers and staff is our priority. Here are some of the measures we are taking to ensure the well-being of our staff, students and Musical Theatre families.
CAMP END / YEAR-END SHOWCASES 2022 - 2023
We fully intend and hope to produce the Camp/Term/Year End Showcase for the 2021-2022 season. As Government regulations are released we will provide more details on what our showcase performances will entail. However, HaoIs musical school reserves the right to cancel or pivot these productions based on the safety guidelines outlined by the province and local municipalities.
As we open our doors, we thank you in advance for complying with these policies and helping to keep our HaoIs musical school safe.
Haois International Arts School will strictly follow all guidelines provided by “Ontario Dance and Performing Arts Studios Workplace Guidelines for COVID-19” & Ontario Public Health and Safety. We are committed to providing a safe space. The following is a list of protocols that you can expect at the studio.
DROP OFF PROTOCOLS
All students are encouraged to enter the Building Entrance through the Front gate, no less than 5 minutes prior to the start of class.
COVID SCREENING
- Our school-specific mask protocols: All students and staff are recommoned to wear masks indoors - in common areas and inside the studios, and while dancing and singing. If eating snacks between classes students can remove their mask but remain at a 2m physical distance from other students, and off to the side during water breaks.
- Physical Disdancing: Stickers and floor tape have been placed throughout the studio to mark one-way pathways and dance areas for students. Staff will ensure that the dancers maintain physical distancing.
- Schedule: We have adjusted our 2021/2022 schedule to allow breaks between classes for staff to sanitize the dance floors and all surfaces. All updated schedule information is available on our website.
- Frequent Cleaning and Sanitation: Strict cleaning protocols will be in place. The facility will be properly sanitized and cleaned thoroughly daily.
- Limited physical contact: Our staff will not be providing any hands-on corrections to students at this time. Teachers will provide verbal feedback, corrections and clear explanations through their coaching, while maintaining physical distancing. Teachers will only provide hands-on corrections that are needed for the safety of students.
WHAT TO BRING TO THE SCHOOL:
Students must arrive dressed in comfortable clothes that allow them to move freely. They will NOT be able to change attire between classes. Indoor/Dance shoes will be put on once inside the studio. Please only bring what you need into the studio. No large bags or extra items/clothing. Students should bring a water bottle with enough water to last the full class.

- Personal belonging brought to the school should be minimized and not shared between individuals. Personal items (eg. Backpacks, clothing, shoes, towels, food, devices) should be labelled or clearly identifiable, reserved for personal use only.
- If you or your family has been exposed to COVID-19 please let the school know immediately by calling or emailing (contact information below).
- Once the student has entered the building, HaoIs staff will direct students to their appropriate studio. Waiting chairs have been placed, socially distant, outside each studio.

- Limited physical contact: Our staff will not be providing any hands-on corrections to students at this time. Teachers will provide verbal feedback, corrections and clear explanations through their coaching, while maintaining physical distancing. Teachers will only provide hands-on corrections that are needed for the safety of students.
- Stay Home if Sick: Students must stay home if they have a fever or are feeling generally unwell. Please contact the studio if you will be staying home. Staff will also stay home if they are feeling unwell. Students are not permitted to enter the facility if the screening questionnaire is incomplete.
- Reduced Class Sizes: Our class sizes have been reduced to maintain social distancing within the studios. Class sizes range from 8-15 dancers, depending on the size of the studio, each student will have their own set of materials in their designated square should their class require. Any shared materials will be sanitized between use, and all materials sanitized prior to the following class.
- All studio spaces are equipped with a hand sanitizer station and Disinfection wipes for material wipe down.
- We have procedures in place should a student show symptoms of COVID-19 during class. They will be escorted by a teacher to our designated isolation area where they will be supervised by a staff member until a parent/guardian can come to pick them up. Staff will follow up with the family regarding COVID testing for tracing purposes.
WHAT TO BRING TO THE SCHOOL:
Students must arrive dressed in comfortable clothes that allow them to move freely. They will NOT be able to change attire between classes. Indoor/Dance shoes will be put on once inside the studio. Please only bring what you need into the studio. No large bags or extra items/clothing. Students should bring a water bottle with enough water to last the full class.
